Whole Person Advocacy and Access Enhancement
Whole Person Care Navigation for the Family
As an extension of your Executive Health team, the whole family can have access to a dedicated whole person care navigation team throughout the year through an enhanced Executive Health program. This includes a Lifestyle-Medicine certified nurse navigator and a licensed mental health professional who work together to provide coordinated, proactive care for the whole family’s well-being.
This enhancement includes:
- Help find care, answer questions and connect to resources
- Provide education on conditions, treatments and preventive health options
- Offer mental and behavioral health counseling and connection to personalized care
- Provide health coaching, goal setting and routine check-ins
- Help find the right specialist or primary care provider
- Serve as a trusted guide for the whole family throughout their health journey
How It Works
- Begin by completing this interest form or emailing EmployerSolutions@vumc.org, and a member of our team will reach out to you shortly.
After your comprehensive physical has been scheduled, a triage nurse will conduct a detailed interview to discuss your existing conditions, family medical history and specific health goals.
- During your visit, you will undergo a full health check during a half-day session, including any additional tests recommended by our triage nurse.
We conduct a same-day review of results with you, including feedback from a doctor, nurse and exercise physiologist.
- You will leave our clinic with a clear understanding of your overall health and personalized care recommendations for the year ahead.
