Requesting your or your child’s medical records (for a child younger than 18 years of age) is now easier with more options.
Request Records Online
VUMC is pleased to now offer patients and legal representatives an online request process for medical records. This may be accessed via the link on the right or through the My Health at Vanderbilt patient portal.
To successfully complete your online request, ensure you have the following available:
- Your photo ID/driver’s license (or a clear picture of your ID that can be uploaded)
- Mobile phone/tablet
- Name(s) of provider(s), types of records needed, location, and dates of service for the records to be released
- If you are requesting records be sent outside of Vanderbilt University Medical Center (VUMC), you will need the full address, phone number, and if applicable, the name of the provider/facility
- If you are requesting medical records about another patient other than your own child, please have the authorizing documents available such as power of attorney, guardianship papers, executor’s documents, death certificate of the patient, or birth certificate of the patient, or other documentation that shows appointment as legal representative.

- Complete the authorization form.
- Fax or mail the completed form to the facility listed below where you/your child received care as outlined below.

Update Patient Identifiers
Life events or other situations often necessitate updating a patient’s name, date of birth (DOB), Social Security number (SSN) or legal sex in the electronic medical record system. While updating this information during an in-person appointment or visit has always been and will continue to be an option, Vanderbilt Health offers an online option to submit the change request for patients or their legal representative.
Appropriate supporting documentation (government-issued identification or legal document) is required to be submitted in order for the change to be made. To get started, complete this online form.
Report a Death
Vanderbilt University Medical Center (VUMC) is often unaware when a VUMC patient dies at a non-VUMC hospital or facility, or elsewhere outside of VUMC. If you would like to make VUMC aware of a patient’s death, complete this online form.
Please have available the following information in order to fill the form out completely.
- Patient's full legal name (and any alternative legal name)
- Patient's address
- Patient's date of birth (DOB)
- Last four digits of Social Security number (or full number), if known
- Legal sex
- Date of death
- Digital copy of death certificate or link to obituary, if available
Alternatively, if you would like to notify VUMC by mail of the patient’s death, send a copy of the death certificate and/or obituary, as well as the patient information details requested for the online form as noted above to the following address:
