What is the MyHeartHealth program?
MyHeartHealth coordinates all the services needed for patients recently diagnosed with a cardiac arrhythmia, including atrial fibrillation (AFib or AF), atrial flutter (Aflutter) and supraventricular tachycardia (SVT). The primary goal of the bundle is to proactively address irregular heartbeats to avoid adverse and costly health care episodes. The bundle includes two electrophysiology (EP) procedures—catheter ablation and left atrial appendage closure.
Who is eligible?

What services are included in the bundle?
- EP/Cardiology visits
- Cardiac testing/monitoring
- Related diagnostic imaging/labs
If receiving an approved procedure, the following services are also included:
- Pre-procedure imaging/labs
- Ablation procedure
- LAAC procedure
- Related admissions for duration of bundle
- Post-procedure ED or observation visits for duration of bundle
- Kardia monitor/BP cuff for home monitoring
What services are NOT included in the bundle?
- Prescription medications
- Unrelated services
- Services by non-MyHeartHealth network providers
How long does the bundle last?
Bundle coverage begins upon enrollment. If your clinical team determines that you will receive conservative treatment or a non-bundle-eligible procedure, your bundle will end once your treatment plan is confirmed, up to 60 days after your provider visit. If you receive an ablation procedure, your bundle will end nine months after the procedure. If you receive a left atrial appendage closer (LAAC) procedure, your bundle will end 12 months after the procedure.
What happens if I want a service not covered in the bundle?
The bundle does not limit your access to any services such as prescription medications. If deemed clinically appropriate by your provider, any services outside the bundle will be covered by your traditional insurance plan. Out-of-pocket costs will be determined by your insurance plan. If you have questions about estimates, contact your insurance carrier.
What happens if I have a cancellation event?
If a cancellation event occurs, your bundle will be terminated, and all future care will be billed to your insurance. Any out-of-pocket costs will be determined by your insurance plan.
Does having a cancellation event disqualify me from future bundles?
No. A cancellation event does not disqualify you from future enrollment in any MyHealth Bundle if you meet specific bundle requirements.
