What's Included

If surgery is determined to be the most appropriate course of treatment, the bundle includes:

  • Office visits pre- and post-surgery
  • Specialist visits for surgery clearance
  • All activities related to the procedure and hospital stay
  • Post-operative rehab

*Please note that this bundle is for non-injury-related (atraumatic) shoulder pain only. Shoulder pain caused by accident or injury is not included in the bundle.

Eligibility and Enrollment

If you are eligible to participate in the MyOrthoHealth – Shoulder Pain Bundle, you must speak with one of our patient navigators to enroll in the program. Eligibility and enrollment requirements vary by employer, and our patient navigators can walk you through the process and help schedule your first appointment.

How It Works

  • Once you are confirmed eligible for the bundle, the patient navigator schedules you for an ortho consult to confirm that a non-surgical pathway for your shoulder pain is the correct diagnosis.
  • If physical therapy is not producing desired results, we will schedule a surgical consult and conduct advanced imaging. If you are determined to be a good candidate for surgery, you will be scheduled for the most appropriate procedure. 
  • Pre-surgery education helps you prepare for the procedure.
  • Surgery will typically involve an overnight hospital stay, or in some cases will be offered as an outpatient procedure. 
  • Following surgery, you will participate in up to 16 weeks of post-operative physical therapy based on the procedure and severity of diagnosis.

Duration

Bundle coverage begins upon bundle enrollment and from the initial patient consultation. Depending on an individual’s treatment plan, the program can last up to 9 months.
