Interdisciplinary Care Program

The team of health care specialists at the Vanderbilt Interdisciplinary Care Program program provides ongoing care for people with complex chronic conditions. These patients have relied on emergency care or repeated hospital stays. We create customized care plans that address your environmental, behavioral, emotional, physical and functional needs. Our goal is to improve your health and quality of life while reducing your need for stressful and expensive emergency care.

We coordinate care with your primary care physician and specialists when and where possible. Our team provides primary and urgent care in a dedicated clinic. We also offer inpatient care in a dedicated unit if you need admission to the hospital.

    We are a key point of contact for our patients' ongoing health care. We work with your primary care provider and other long-term providers for regular and urgent medical needs. Our outpatient clinic offers a familiar, comfortable place when our patients need care they might otherwise seek from the Emergency Department.

    Our team includes doctors, nurse practitioners, a nurse case manager, a social worker, a pharmacist and nurses who work together to offer care tailored to each patient’s medical challenges.

    Patients cared for by the Interdisciplinary Care team who need hospitalization will be admitted to our unit in Medical Center North when possible.

    Patients can reach a care team member at any time by phone or text. We check on patients immediately after hospital discharges and between regular appointments.

    Follow-up after hospitalization

    Patients will be seen by providers after a hospitalization to ensure the patient continues to improve after discharge, answer questions, ensure the right medications are prescribed and meet any other needs.

Tests, Treatments and Services

The Vanderbilt Interdisciplinary Care Program does not treat patients while they are in critical care units (medical, surgical or cardiac) or when patients are admitted at the Vanderbilt Psychiatric Hospital. Our team manages patients when they are transferred to the general medicine unit.

Because our patients have a wide range of chronic conditions and symptoms, we offer many tests and treatments typically offered by urgent care clinics or the Emergency Department. These include but are not limited to:
