My appointment with Dr. Pishka was professional, but I left feeling that my case was approached primarily through a standard insomnia protocol rather than a more individualized evaluation. I have a complex history that includes severe treatment-resistant depression and long-standing insomnia that has not responded to numerous medications and therapies over many years.During the visit, the primary recommendation focused on sleep hygiene and reducing time in bed. While I understand that these approaches can help many patients, my insomnia is not typical. Without clonazepam I often sleep very little or not at all, and many other medications have either failed or significantly worsened my depression. Because of this, applying a general protocol without more discussion of my unique history felt concerning.I would have appreciated a deeper conversation about how to manage severe, treatment-resistant insomnia in a way that carefully weighs risks and benefits. In complex cases like mine, a strictly protocol-driven approach may risk doing more harm than good if it does not fully account for the patient's individual history and previous treatment responses.
